Mental Health Practitioner - Outreach CAMHS

Somerset Partnership NHS Foundation Trust, in collaboration with the Somerset Clinical Commissioning Group (SCCG) and NHS England, are developing their current outreach team to provide an enhanced, extended hours service to young people who require intensive community support. For some young people, this work may involve supporting them and their families/carers to prevent admission and may also involve facilitating earlier discharge from hospital. The team currently operates from a multi-disciplinary approach with nursing, psychology, psychiatry and occupational therapy all contributing to providing care and treatment plans. Interventions primarily take place in client’s homes, community settings and via in-reach to the local district general hospitals and inpatient psychiatric wards. The team support young people with a breadth of difficulties from anxiety and depression, to emotional dysregulation and high risk taking behaviours to Eating Disorders.

As part of the development process the team are looking to recruit enthusiastic clinicians and practitioners, with a background from any discipline in a mental health setting, to add to the expertise of the team. Within the developmental plans the team are looking at providing intervention from 8am to 8pm, seven days a week, operating on a shift basis. Additional support for the team will be provided by Psychiatric Liaison Practitioners, the on-call Consultant and Manager, the developing Community Eating Disorders Services (CEDS) and close links are in place and will be reinforced with the district general hospitals. The team is moving towards a core therapeutic delivery model grounded in Dialectical Behaviour Therapy and training and development opportunities will be available within this approach.
