Job overview

  • Hold a clinical caseload, providing comprehensive nursing assessments to determine the appropriate category of care required for a care home placement, and to support the Continuing Health Care and NHS Funded Care process.
  • Foster robust, open and positive working relationships with all partners, in particular, Local Authority and other mental health services and the 3rd sector, ensuring that all partners are treated with dignity and respect.
  • Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management.

Main duties of the job

The post holder will provide comprehensive nursing assessments to determine the appropriate category of care required for a care home placement and to support the Continuing Health Care and NHS Funded Nursing Care process.

The post holder will develop and maintain a collaborative approach to the fair application of both NHS Funded Nursing Care and Continuing Health Care criteria.

The post holder will encourage joint working and actively work towards improving outcomes for patients via the process and provide accurate advice and information to the multi-disciplinary team and families regarding the outcome of the nursing assessment process.

The post holder will frequently work as a Lone Worker.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Experience

Essential criteria
  • Significant experience of successfully operating in a patient centred environment.
  • Experience of working in multi-disciplinary teams.
  • Experience of drafting reports and correspondence.
Desirable criteria
  • Experience of working within a multi-agency community mental health setting.

  • Excellent oral and written communication skills.

Qualifications and/or Knowledge

Essential criteria
  • Registered Mental Health Nurse
  • Post Registration qualification to postgraduate Diploma level, or possess skills, knowledge, training and experience to equivalency.
  • Excellent clinical knowledge of specialist area.
  • Good knowledge of NHS Funded Nursing Care and Continuing Health Care
Desirable criteria
  • Knowledge of Escalating Concerns Policy

  • The role is community based, therefore the ability to travel throughout the UHB locality in a timely manner is essential to the role.
