Clinical Pathway Nurse

Pennine Care NHS Foundation Trust provides specialist mental health services and community services for more than one million people in Bury, Oldham, Rochdale, Heywood, Middleton, Stockport, Trafford and Tameside and Glossop. The Trust’s Headquarters is based in Ashton-under-Lyne, ten miles north east of Manchester city centre.

In July 2008 we became the 100th NHS Foundation Trust which provides greater freedom to tailor our future plans to best meet the needs of the communities we serve and be more accountable to local people.

The Trust provides support to all new starters, in patient facing roles bands 1 to 4, to successfully complete the Fundamental Care Certificate standards relevant to their role. The certificate provides knowledge and skills to enable staff to perform safely and effectively.

All new starters to Pennine Care NHS Foundation Trust who are appointed to a post which requires a Disclosure Barring Certificate check (DBS) will be recharged for the full cost of the check. Substantive staff can choose to spread the cost over their first three months’ salary or, if preferable, pay the full amount from their first months’ salary. Members to the Trust bank must repay the full amount from their first pay. The Trust encourages all staff and bank workers to registered with the DBS update service at an annual cost of £13, for bank members this is obligatory. The cost of an enhanced DBS check is £44 and for a standard check £26, both attract an administration charge of £3.50.

The Clinical Pathway Team (CPT) is the first point of contact to Pennine Care’s rehabilitation services. The team has a specific responsibility for the assessment of male individuals referred to the Trust’s Low Secure Services and all referrals to PICU.

The team is based at Heathfield House in Stockport and is led by the Clinical Pathway Nurse Specialist and consists of 2 nurses with consultant support.

The team also provides a risk clinic to ensure that individuals are cared for in the most appropriate environment.

Role Summary:

The Clinical Pathway Nurse’s role will be to support the Clinical Pathway Nurse Specialist (CPNS) in the assessment of individuals referred to Trust’s Low Secure Units (LSU) and in the management of the referral process of the Psychiatric Intensive Care Unit (PICU).

This will include:
• The monitoring of individuals in Out of Area Placements (OAT’s) both for LSU and PICU.
• Specialist assessments for LSU and PICU, contributing to the formulation of risk plans
• Completion of collaborative pre admission work in preparation for their admission to the LSU services
• Working closely with RHSD consultants across the directorate

An integral part of the role will be to provide detailed assessments of the needs of complex service users placed in a range of secure facilities; prisons; acute mental health services. This supports the team in identifying the most appropriate and least restrictive service required to meet their needs whilst effectively managing their risks. The role within the CPT provides a key contact point for referrals to both LSU and PICU in supporting services.
