We are looking for an experienced, enthusiastic, pro-active, self-motivated nurse to join the team Adult Cardiorespiratory Enhanced and Responsive Service (ACERS) as one of the key links in two exciting new projects in the City and Hackney Neighbourhoods. 

Essential to success in this role is a strong background in cardiovascular long term conditions management.  With relevant training and your excellent communication skills, you will educate, support, and empower patients and carers for increased confidence in self-care of their long-term condition.  Experience of working in the community is desirable. This post will let your personal and clinical skills blossom as you set new ways of working in place to really make a difference.

The project runs initially for 12 months. 

Main duties of the job

The Post Holder will lead the City and Hackney Primary care Blood Pressure Monitoring at home programme.  Onboard eligible patients to the programme, provide education and support.  Help monitor their blood pressure and assist the GP with the titration of antihypertensive medications for his/her caseload of patients.  Be a subject expert in the management of hypertension and liaise with and support other stakeholders as relevant.  Run the Primary Care Network (PCN) hypertension engagement programme, delivering at least one event in each of the City and Hackney Primary Care Networks.  Work with each PCN to design an event relevant to its local population and needs in collaboration with other key stakeholders.  Report on the outcomes of each engagement event and support further business case and service development. To support the training of other health care professionals in the management of hypertension .

The post holder will work alongside the ACERS Heart Failure Team, the ACERs Service are a team of dedicated healthcare professionals providing care for patients with chronic cardiorespiratory conditions within the community and hospital.
