There is a fantastic opportunity for an experienced and enthusiastic Health Visitor to join our friendly, supportive team in West London. 

We work with families in one of the most diverse boroughs in London to give children the best possible start in life by providing the most up-to-date evidence-based support. This is a newly revised role to focus on the High Impact Area of health literacy, reducing accidents and managing minor illness. 

As part of our skill mix team, you will contribute to the delivery of the national Healthy Child programme across Hounslow, working closely with partners, such as hospital providers,  GPs, children’s centres and social care providing seamless care to the population of Hounslow. You’ll also have an important role helping to recognise and protect vulnerable children.

To work in a public health role in the provision of a comprehensive service to facilitate the improvement and maintenance of the health and wellbeing of children, individuals and families.

The post holder will manage the Advice Line team and lead on content of the Healthier Together website to ensure the local population is able to access evidence based health information including managing minor illness.

The post holder will work with commissioners and local providers on reducing A/E attendances .

The post holder will ensure appropriate referral and liaison between hospital services, walk in centres, Urgent Care Centre and Advice Line Health Visitors and 0-19 team.Acting as a professional resource and link between Acute, Community and Primary Care services establishing good communications within these areas including wards, out-patients, A&E, SCBU and maternity services.

  • Evidence of continual professional development

  • Masters Level Advanced Practice qualification or equivalent

Experience

  • Years post SCPHN experience and delivering Healthy Child Programme
  • Safeguarding children knowledge and experience
  • Experience of working with other agencies, especially social services and commitment to inter-agency working
  • Awareness of High Impact Areas
