In 2008 the Government launched the End of Life Care Strategy recommending provision of care 24 hours a day, 7 days a week which can be easily accessed and responsive to all palliative and end of life care patients regardless of their place of care. We are looking to recruit 2.0 wte Band 5 Community Nurses for the Hillingdon SPA and PONS service within the London Borough of Hillingdon, you will be supported by Band 7 nursing and Band 4 administrative roles.
We are looking for innovative, enthusiatic and dynamic nurses to join our team whose aim is to:
• Improve the coordination and continuity of care and support for patients and their families/carers.
• Enable responsive overnight nursing care for people in the community who are at the end of life
• Introduce a single contact telephone number, to enable direct access to services and support for health and social care professionals, patients and their families/carers.
We are looking for expert nurses with experience of looking after patients who are nearing the end of their lives and who enjoy working in an ever changing and innovative environment. Applicants should have a creative, imaginative and flexible approach to service delivery and quality improvement and have excellent interpersonal skills with experience of working in the community, hospital or hospice environment with associated palliative care experience.
You must have appropriate UK professional registration.
