NELFT are seeking an enthusiastic and highly motivated full time Integrated Care Assistant (ICA) to complement our skill mix within CHSCS Havering. The ICA will support with the provision of holistic health care as delegated within own level of agreed competency, to patients, families/Carers in their own homes, to include residential care homes.
The Integrated Care Assistant (ICA) will support in the provision of assessments and treatments, implementing and evaluating programmes of care with supervision, under the line management of the Team Leader. The ICA works under the supervision of and alongside the nurses and therapists to carry out rehabilitation programmes and remedial group activities within the community.
The ICA will assist the team to liaise with the MDT, including supporting and referring on to other health services, social services, voluntary and independent sectors as necessary.
The role encompasses working in partnership with the patient as directed by the care plan to achieve optimum levels of self-care, and observe record and report all changes to other members of the team within identified time frames. Applicants need to demonstrate a commitment to the provision of high quality multi professional care.
The ideal candidate must demonstrate evidence of NVQ level 2 in Care or equivalent, with one years’ experience at Band 2 or equivalent in a care setting. Commitment to working as part of a multi- disciplinary Primary Health Care team is essential as is excellent verbal and written communication skills.

This position also involves working some weekends & some bank holidays.
At NELFT we set very high standards to ensure we provide quality services. We require prospective employees to carry out an online literacy and numeracy test equivalent to NVQ Level 2.
