Are you a qualified District Nurse looking for an opportunity to develop and lead nursing services to people in their own homes? Would you like to be part of a team that is rapidly developing joint working with our partners in other health, social and non-statutory care providers? Then we would welcome your application for this pivotal role in our Community Nursing Team.
You will need to be motivated with a wish to excel in patient care. Our services are working with our partners towards seamless patient pathways using the Whole Systems Integrated Care approach.
The post holder will be part of an integrated team responsible for the planning, implementation and evaluation of nursing care needs to patients within the community.

You must have appropriate UK professional registration.
