Do you want to be part of a team that is influencing change and transforming the delivery of mental health services for people with personality difficulties?

Personality and Complex Trauma Team (PACT) are continuing to grow and expand across the county of Lincolnshire. We are looking for enthusiastic qualified practitioners (suitable for Nurses, Social Workers, AHP) to join our team.

Our current team describe why they are committed to remaining within PACT: Feeling safe, having time and space to deliver high quality care, truly small caseloads and high levels of clinical supervision and reflective practice. PACT practice with compassion and kindness to instil hope and empowerment of the people we support to feel in control of their treatment and inspired to live a meaningful life.

We offer Psychologically Trauma Informed models of care to people with personality difficulties. 80% of people with personality difficulties would no longer meet the diagnostic criteria following evidence-based interventions.

Prif ddyletswyddau'r swydd

Manage caseload and service delivery in accordance with Service/Trust requirements, ensuring continuity of care.

Successfully complete training and deliver all aspects of Structured Clinical Management that is adherent to the model & SCM competency framework.

Provide high standard of clinical care that ensures safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.

Undertake risk assessment and risk management for relevant individual clients, along with providing advice to other professionals on this subject.

Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.

Enable individuals to develop independent living skills and live in the community.

Work collaboratively with a wider range of professionals and agencies as part of a multi-disciplinary team. Ensuring effective and appropriate clinical decision making.

To participate in Clinical Governance activities, including induction, supervision, personal development review, health and safety, risk management and audit.

Maintain up to date, accurate and holistic clinical records to the highest standards.

Protect people from abuse, neglect and harm.

Support development of self and others and work within co-production environment to develop service delivery.

To provide high quality evidence based treatment to a caseload of service users including taking on the role of lead professional for individualised care and support plans.

To deliver groups as part of the treatment pathway and within other services to develop their pathways

To ensure continuity of care by:

  • Providing client with contact numbers
  • Planning appointments and maintaining reliability and consistency in contact
  • Maintaining contact during episodes of in-patient care and involvement in discharge planning
  • Contact and communication with GPs & other Primary care staff
  • Out of hours arrangements/cover for Annual Leave
  • To manage the transfer of a client’s care to another team member or service
  • Attend appropriate clinical meetings either in the community or other settings in relation to the client’s care.

To successfully complete Structured Clinical Management Training and deliver the socialisation period, SCM groups and SCM 1-1 sessions, that are adherent to the model and successfully working to the SCM competency framework and Knowledge and Understanding Framework for working with people with personality difficulties

Liaise with a wide range of other professionals and agencies.

To ensure regular attendance at managerial and clinical supervision sessions.

Support the development of the peer support workers and ensure they are equally included in design and delivery of relevant aspects of the service and promote coproduction

To act as practice educator for students and provide clinical supervision when required.

Contribute to the safe and effective day to day operations of the service including the handling of clinical enquires.

Contribute to the team’s engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.

To complete assessments, formulations and implement plans for the formal psychologically informed and psychological treatment .

To provide advice to staff working with the client group across a range of agencies and settings

To maintain up to date records in a timely and accurate manner.

To carry out duties in relation to local, regional and national initiatives in developing services in the specialist area as opportunities arise.

To triage referrals to the specialist treatment pathway by assessing and advising on eligibility criteria.

To undertake risk assessment and risk management for relevant individual clients and to provide advice to other professions on risk assessment, formulation and risk management.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.

To work flexibly as required to meet the needs of the service and the service users, may require some evening and weekend working.

To delegate aspects of the patient treatment plan to other members of the wider clinical team, whilst maintaining overall responsibility for patients care, where appropriate.

To foster a culture across the system and with people accessing the service of Trauma Informed Approaches that promote empowerment, autonomy and hope.

To assess carers and families needs and develop, implement and review programmes of support for carers and families.

Protect people from abuse, neglect and harm.

Enable individuals to develop independent living skills and live in the community.

To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Team and with external agencies.

To maintain accurate and timely clinical records and to co-ordinate and monitor those of the team.

To adhere to relevant codes of professional conduct and ethics, plus associated legislation.

To develop evidence based clinical practice with regard for the 5 year forward for mental health

To demonstrate empowering leadership skills within the Team and seek opportunities in local and national area as to promote and develop the profession.

To participate in Clinical Governance activities, including: induction, supervision, personal development review, health and safety, risk management and audit.

To undertake specific project work or any other duties as negotiated within the Team.

Supervise health and social care staff in the support and monitoring of clients programmes of care managed within the team.

Develop effective and supportive links with other health and social care staff. To create networks that improve the pathway of care for clients referred to the Community Team.

Participate in all clinical governance and audit developments including post registration education, training and continuing professional development.

To actively engage in health promotion

To share the task of chairing meetings and taking minutes with other members of the team.

You may occasionally be asked to undertake such other duties that are deemed necessary in order to support business continuity within the Trust. This will only include tasks where you are appropriately trained.

Experience

  • Experience of managing and care coordinating a caseload of community based clients with a range of mental health problems or evidence of transferable experience from inpatient environments
  • Experience of research-based/reflective practice and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence-based practice
  • Working within a multi-disciplinary and multi-agency environment
  • Experience of working with service users with personality disorder/complex clinical needs, self-harming behaviour, complex and challenging presentations
  • Knowledge of clinical governance and audit
  • Knowledge of relevant Professional and Clinical legislation, specifically for Personality Disorders
