Every day working as a SCPHN is different, including identifying need, planning and implementing interventions as well
We have a hybrid working model ensuring that we provide care to children and young people in the right place at the right
Our SHOP is the front door to the service where all referrals and enquiries are triaged, assessed and next steps planned
The ideal candidate will have:
* A SCPHN qualification (SN or HV wishing to develop their knowledge and skills working with 5-19 yr olds)
* Flexible approach to meet the needs of the service.
* Be an effective communicator on all levels, with the ability to provide advice and resolve issues.
* Be able to work under pressure and to deadlines.
* Be organised and meticulous, with an eye for detail.
* I.T literate.
* Car owner/driver essential, subject to the provisions of the Equality Act (2010), as driving around the county will be

Experience

- 1) Minimum of 2 years’ experience in area of practice 2) Practical experience in managing health related projects/complex caseloads 3) Supporting others (mentor / educator / supervisor) experience
Knowledge

- 1) Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services 2) Evidence of continued relevant professional development and implementation in practice within the last 12 months
