We are looking for an enthusiastic and adaptable End of Life Specialist Community Nurse to join our caring and progressive team in Canterbury.
You will work as part of a close-knit team who are dedicated to the health and wellbeing of people in Canterbury. Canterbury is made up of a diverse patient group with complex needs.
Your team will be closely aligned with local Primary Care Networks, to make sure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.

Community Nurses provide complex nursing care to people at home and in residential care homes.
As a End of Life Specialist Community Nurse, you'll
- assess the healthcare needs of patients and families
- monitor the quality of care they're receiving
- be professionally accountable for its delivery
Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.
As a Band 7 End of Life Specialist Community Nurse you will work as part of a team to keep people safe and well at home. You will use your clinical judgment to support evidence-based care planning for people living at home, in care homes or other settings and make sure that people who use our services have a good experience.

Qualifications

- Registered health care professional (NMC registration) / minimum of five years’ experience in health improvement environment.
- Health related degree and evidence of relevant post graduate development at Level 6/7.
- V300 Prescribing and Advance Practice Skills.
- Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: SPDN, Mentorship, Coaching, Clinical Expertise Pathway).

- Advance Practice Skills and Clinical Reasoning qualification.
- Management or supervisory qualification/ programme of study.
Experience

- Minimum of five years’ experience in area of practice.
- Experience of leading and managing teams.
- Experience of supporting the development of protocols and service development plans with practical experience of change management.
- Practical experience in managing health/ team related projects.
- Experience of managing complex caseloads.
- Experience of supporting and being a role model to others (mentor / educator / supervisor) through clinical supervision/ 1:2:1 and personal development plans.
- Experience with the use of Microsoft Office/ eRoster/ Appraisals/ Training Matrix/
Knowledge

- Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across health care services.
- Evidence of continued relevant professional development and up-to-date implementation in practice.
- Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations
- Experience of supporting and coaching others (mentor/ educator/ supervisor) to achieve a positive mind set and promote health and wellbeing.

- Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social inequality.
