We have an exciting opportunity for a Band 6 Case Manager to join the Canterbury Locality.
This role requires the post holder to manage a defined case load and utilise their clinical and leadership skills to assess, triage, plan, delegate, organise and evaluate complex packages of health and social care. The successful candidate will need to be able to first line manage their nominated team, also having the ability to act up in the absence of the Clinical Lead Nurse.
The Case Manager will support the Clinical Lead Nurse and locality managers in achieving the key outcomes of patient safety and quality across the case load. The successful candidate will be motivated and engage in the Trusts vision.
Applicants should be a Registered Nurse with community experience and evidence/experience of team leadership skills. A relevant degree is essential or willingness to undertake will be expected. As part of the Urgent Care Agenda applicants will be expected to work flexibly between 0800-2200 hours.
