This vacancy is for a Health Care Support worker within the Derwen Integrated Team for disabled children. The successful post holder will provide support to children and young people and their carers to implement programmes of care of interventions designed by the Children with Disabilities Nurses and clinical psychology. Maintains records and programmes aims and objectives, reporting back to the responsible clinician and maintaining a positive relationship.with families.

The ability to speak Welsh is essential for this post
