Job overview

East Sussex Healthcare Trust has exciting opportunities for Registered Staff Nurses on our Discharge Team based in Conquest Hospital, and we are looking for highly motivated, dynamic and passionate individuals.

Are you looking for a change of pathway, we are developing changes in the discharge or our patients. Can you be flexible and work autonomously alongside junior staff? Would you like to join our small friendly developing team, we would welcome the opportunity to meet with you.

You will support ward staff and their multi professional teams in facilitating effective patient discharge as an integral part of care management within the Trust. You will ensure best evidence based practice in planning safe and timely discharge,
