Barrow's  ICC's core team is a small multidisciplinary team that supports and promotes collaborative working between health, social care, other agencies and the community.  In addition the team attends external health promotion events and we even organise our own too, occasionally you would be expected to support these.

A permanent opportunity has arisen for a  Care Coordinator to join the team.  We are looking for a friendly and proactive team member to assess, plan, implement and evaluate evidenced based patient care, liaising with the multidisciplinary team within a community setting, supporting our residents  to live independently. 

Based within Fairfield Lane Community Offices with travel across Barrow-in-Furness and surrounding area.

The ICC has an extensive network of local service providers which you will utilise to implement the anticipatory care plans. Your role will involve liaising closely with other providers from both statutory and voluntary agencies.    

We are looking for a Care Coordinator who is  

  • 1st level registered nurse or allied health professional
  • Passionate about caring for people in their own homes 
  • Focussed on preventive care and admission avoidance 
  • Independent, resourceful, and proactive

As a team we develop and implement comprehensive packages of information, education and support for patients and their carers/relatives, which may include attendance at local public events/health festivals.  The role offers opportunity to develop and maintain an area of clinical expertise.

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life-threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
