Job overview

The post holder will have responsibility for developing and supporting this speciality service to provide equitable care to patients presenting with Lymphoma Cancer and act as a resource to carers, the Multi-disciplinary colleagues and the primary health care team. This will be achieved by the post holder ensuring a smooth transition of care from initial referral through diagnosis, treatment and beyond treatment into the survivorship stage, establishing links between the acute and primary care group, supporting both patient and carers to ensure a robust stratified follow up process for cancer patients.

The post holder would liaise with services within Wales and with other key partners such as Health Boards, Primary Care, Voluntary Services, Social Services and Hospices. The post holder will provide expert nursing advice and support to patients and their families at all stages of disease.

The main responsibilities of the role will be related to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence. 

A Non medical prescribing qualification would be desirable but can be taught.

 

This post is a secondment for 12 months due to meet the needs of the service.

Main duties of the job

The duties required for this role are:

  • Develop the knowledge base of other nurses and health care professionals caring for patients with myeloma
  • Initial patient assessment and initiating a care plan for patients in partnership with the wider multidisciplinary team (MDT).
  • Supporting patients whilst they are undergoing diagnostic tests
  • Assessing prescriptions to maximise patient management.
  • Providing patients, their families and staff with expertise (in-depth knowledge and understanding of Lymphoma)
  • Undertaking medicine management for lymphoma patients.
  • Initiating and implementing standards of nursing treatment and care for Lymphoma patients
  • Developing new approaches to running services
  • Undertaking strategic planning to incorporate specific speciality needs and evidence-based practice
  • Providing nurse-led clinics as appropriate such as pre and post transplant and inpatient/outpatient
  • Undertaking HN

Other duties will include

  • To request and organise chemotherapy treatments, stem cell collection and transplant work up.
  • To liaise with community palliative care teams and other community based agencies to provide community support and specialist care for those that require it.
  • To be actively involved in myeloma clinical trials at UHW and to be the main point of contact to facilitate recruitment of myeloma patients from across South Wales into appropriate trials in conjunction with the clinical trials team.
  • Network locally and nationally to ensure best practice

DUTIES AND RESPONSIBILITIES

JOB SUMMARY/JOB PURPOSE
Working as an autonomous practitioner devise, implement and follow up specialist 
treatments across the disease spectrum for patients within a specific group. To be a 
skilled practitioner providing expert clinical care using an extensive theoretical 
knowledge base and practical experience to underpin the role, while acting as a role 
model for the delivery of expert care.

To line manage staff in specialist area including staff from other disciplines.
To lead research and audit within given speciality internally and externally to the UHB


Clinical and Patient Care Responsibilities
To work as an autonomous expert practitioner
To provide up to date evidence based care.
To be an expert resource, providing expert clinical advice to all members of the interdisciplinary team, patients, carers and other agencies.

To utilise advanced nursing skills and extensive knowledge base to undertake specialist assessments, critically analysing complex information to make informed decisions regarding appropriate treatment/intervention to ensure the delivery of highly specialist care.

Clinical Practice

To autonomously manage individual caseload, using expertise in devising, 
providing and implementing highly specialised programmes of clinical care 
spanning primary, secondary and/or tertiary care.

Research
 To be able to understand, analyse and disseminate current research and be able to critically analyse its significance to practice. Implementing findings to improve care for service users where needed and share up to date practice with other nurses/health care professionals in the clinical area

 To participate and/or co-ordinate UHB, local and national research activity within the specialist area of practice. Ensuring GCP (Good Clinical Practice) certification is 100% up to date

Management
 To take the role of Lead Clinical Nurse Specialist within specialist service

 To be responsible for the immediate identification and response to clinical risk issues within specialist area by instigating measures/action plans to protect patients and staff health and safety.
 To maintain accurate, easily accessible records of all clinical incidents (including the monitoring of trends). 
 To agree action plans (when appropriate) to prevent recurrence of critical 
incidents.
To raise concerns regarding clinical risk/poor practice within specialist area to Lead/Senior Nurse/Directorate Manager/Clinical Director/Clinical Risk 
Department.
 To comply with the University Health Board policies and protocols. 
 To present clinical risk issues at Health Board Quality and Safety meetings when necessary.
