Passionate about stroke care, want to work in the community? A rare and exciting opportunity has arisen within the community stroke nurse team within Buckinghamshire.

The team is expanding its cover to become countywide. The nurses provide advice, information, onward referrals and support to stroke patients throughout Buckinghamshire. We are looking for a new team member to work alongside our existing team.

Following the individual throughout the stroke pathway you will be a point of contact and help facilitate a seamless service. Individuals are followed for up six months post stroke and you will be responsible for coordinating their support and completing six month stroke assessments as part of the stroke sentinel national audit programme. You need to be self-motivated, enthusiastic about stroke and have the ability to work independently and as part of a team. We have strong links with our acute stroke unit (which has been graded A by the stroke sentinel audit for the past year) and community stroke teams.
